Health, routines, preferences, likes and dislikes, and what you want help with.

We check safety
The home, mobility, medication and any risks.

We agree a plan
Visit times, tasks and costs – written into your care plan.
Useful things to have ready
- A list of medication
- GP and pharmacy details
- Any hospital discharge letters or care plans
- Times of day help is needed most
After the assessment we confirm everything in writing and your carers are introduced before care starts.
